Betel leaf, also known as betel pepper, betel vine, wild pepper, among other names, scientifically named Piper betel L. (Piper siriboa L.), belongs to the Piperaceae family. Betel leaves originate from Southeast Asia and are extensively cultivated in India, Sri Lanka, Vietnam, and Malaysia. In Vietnam, betel leaves are grown almost nationwide.
Betel leaf is a climbing plant with slender stems. Its leaves are alternate, petiolate, measuring 1.5 - 3.5cm in length; leaf blades are broadly ovate, 10 - 13cm long, 4.5 - 9cm wide, heart-shaped at the base (for the lower leaves), tapering to a pointed tip, visibly containing numerous tiny oil glands when viewed under light, typically with five veins. In Vietnam, two common types of betel leaves are the fat betel leaf, which is broad and easy to cultivate, and the cinnamon betel leaf, smaller in size, possessing a spicy taste, more favored in betel chewing traditions.

The composition of fresh betel leaves is identified to include water (85-90%), protein (3-3.5%), fat (0.4-1%), carbohydrates (0.5-6.1%), fiber (2.3%), essential oils (0.08-0.2%), and tannins (0.1-1.3%). Among these, the active components primarily consist of essential oils and secondary metabolites (saponins, phenols, ancaloit, amino acids, tannins, flavonoids, steroids...). Many studies have revealed that betel leaves possess strong antioxidant, antibacterial, and anti-inflammatory properties.
The phenolic compounds in betel leaf extracts have been identified by scientists as the main contributors responsible for its antibacterial capability.Betel leaves' antibacterial potential is evident in their ability to eliminate most disease-causing bacteria such as E. coli, Streptococcus pyogenes, and S. aureus. Betel leaf extracts have also shown considerable improvement in conditions like itching, inflammation, pus-filled pimples, and ulcers. Lakshmi Sriyani Arambewela and colleagues investigated the effectiveness of phenolic extract-infused creams for treating dermatological conditions, demonstrating a significant reduction in symptoms for patients suffering from "vicharchika kushta."
Arani Datta and associates reported the leaf's resistance against most gram-positive and gram-negative bacteria causing skin diseases, including Pseudomonas aeruginosa, Klebsiella pneumonia, Proteus vulgaris, and Staphylococcus aureus. This study revealed that betel leaf extracts exhibit effective antibacterial action by inhibiting MurA, which causes bacterial cell wall destruction, due to hydroxychavicol and allylpyrocatechols.
Moreover, betel leaf extracts demonstrate excellent antioxidant abilities due to their relatively high phenolic content. Sundang and collaborators identified that betel leaf extract's antioxidant capacity surpasses that of vitamin E, butylated hydroxytoluene, catechin, and similar to ascorbic acid (Vitamin C). Betel leaf extracts can eliminate free radicals such as HO, superoxide radicals, and hydroxyl radicals, reduce protons, break down the chain of free radicals caused by hydrogen atoms, enhance the cellular oxidative system, and increase glutathione antioxidant levels by 50%.
