INTRODUCTION TO BETEL LEAF

Betel leaf, also known as betel, betel pepper, betel vine, or sirih, scientifically named Piper betel L. (Piper siriboa L.), belong to the Piperaceae family. Betel leaf originate from Southeast Asia and are extensively cultivated in India, Sri Lanka, Vietnam, and Malaysia. In Vietnam, betel leaves are grown almost nationwide.

Betel leaves are a climbing plant with smooth stems. The leaves grow alternately, with petioles having sheaths, measuring 1.5 – 3.5cm in length; leaf blades are oval-shaped, measuring 10 – 13cm in length and 4.5 – 9cm in width, heart-shaped at the base (for basal leaves) and pointed at the tip, with numerous tiny oil-containing dots visible under a microscope, typically with five veins. In Vietnam, there are two common types of betel leaves: fatty betel and cinnamon betel, with fatty betel having larger leaves and being easier to cultivate, while cinnamon betel, with smaller leaves and a spicy taste, is more favored in betel chewing customs.

ANTIBACTERIAL PROPERTIES OF BETEL LEAF

The composition of fresh betel leaves is determined to include water (85-90%), protein (3-3.5%), fat (0.4-1%), carbohydrates (0.5-6.1%), fiber (2.3%), essential oils (0.08-0.2%), and tannins (0.1-1.3%). Among these, the active components are primarily essential oils and secondary metabolites (saponins, phenols, alkaloids, amino acids, tannins, flavonoids, steroids, etc.). With these chemical constituents, numerous studies have shown that betel leaves possess strong antioxidant, antibacterial, and anti-inflammatory properties.

The phenolic compounds present in betel leaf extracts have been identified by scientists as the main responsible components for betel leaves’ antibacterial activity.

The antibacterial ability of betel leaves is demonstrated by their ability to inhibit most pathogenic bacteria such as E. coli, Streptococcus pyogenes, and S. aureus. Betel leaf extracts have also been shown to significantly improve conditions such as itching, inflammation, pustules, and ulcers. Lakshmi Sriyani Arambewela and colleagues conducted research on the efficacy of phenolic extract supplements from betel leaves in treating skin diseases, finding that patients with “vicharchika kushta” disease significantly reduced their symptoms after using the ointment.

Arani Datta and colleagues reported on the ability to resist most gram-negative and gram-positive bacteria causing skin diseases such as Pseudomonas aeruginosa, Klebsiella pneumonia, Proteus vulgaris, and Staphylococcus aureus. This study also demonstrated that betel leaf extract provides good antibacterial efficacy by inhibiting MurA, which destroys bacterial cell walls, through hydroxychavicol and allylpyrocatechols.

Furthermore, betel leaf extract also demonstrates excellent antioxidant capabilities due to its relatively high phenolic content. Sundang and colleagues have identified the antioxidant capacity of betel leaf extract solution to be higher than that of vitamin E, butylated hydroxytoluene, and catechin, similar to ascorbic acid (Vitamin C). Betel leaf extract can eliminate free radicals such as HO, superoxide, and hydroxyl radicals, scavenge protons, and break down free radical chains formed by hydrogen atoms, promoting the intracellular oxidation system and increasing glutathione antioxidant by 50%.
